Application & Registration:
How to apply/register for Insurance?
Registration and Application for insurance is to be transacted online by:
1) Accessing our website at Trigon Insurance Brokers Ltd.
2) Select>: [Services]->Little League Baseball
[Registration and Application for Insurance]

Follow online instruction and complete all questions.
What documents are required for the Application/Registration?
- A signed Application Form for your Registration for insurance;
- A blank sample copy of your Player Registration Form, with the "Hold Harmless" condition, signed by all players, their parents or guardians;
- A copy of your League’s recent “Financial Summary” indicating revenue and expenses; and
- A list of your present Directors and Officers.

Claims:
Who do I claim insurance from?
If the Claimant (or the Parent) has group insurance benefit through employment, your Sports Group Insurance Program is an excess accident benefits policy. The plan will pay only the amount of expenses that are applicable to this plan and are not eligible with any other heath care plans either provincial, personal and/or employment group insurance plan.
When do I have to report a claim?
Claims notices must be received within 30 days of the accident date.
How long do I have to submit my claim statements?
Statements, Invoices and/or Receipts on paid items for claims, a copy must be submitted within 90 days, which indicate:
- Patient's Name
- Type of Service or Purchase
- Date of each Service or Purchase
- Amount Charged for each Service or Purchase
Dental and/or Physician's Statement must be completed and signed by the attending Dentist or Physician. Dentist and Physician normally have their standard statement for use; otherwise:
- Download a copy of physician statement
- Download a copy of dentist statement
to save these files, right click on the link, choose "Save Target/Link As."
Coverage:
Directors & Officers Liability
Coverage for Directors & Officers Liability will not be provided if the following are not received by our office.
- A copy of your League’s recent “Financial Summary” indicating revenue and expenses; and
- A list of your present Directors and Officers
Equipment and Property covered?
This Group Insurance Program does not cover property insurance for equipments, contents or facility coverage. Detail information can be submitted to Trigon Insurance Brokers Ltd. for quotation. Please note: minimum policy premium and Deductible $1,000 apply, for specific location premises only.
Is vehicle damage by "fly-ball" covered?
Liability coverage for damage to property, such as vehicle windshields, does not provide payment unless negligence is involved. Damaged vehicle is to report to the vehicle's own insurer for claims.
Tournament:
Playing in a Tournament covered?
Your Team is covered when playing in a tournament, anywhere in North America, for Liability and Accident coverage, except that Non-member players are not covered. Liability for hosting a tournament can be extended; detail information must be submitted to Trigon Insurance Brokers Ltd. in confirming coverage and additional premium charged.
Travel Insurance:
Travel Insurance - is it covered?
Travel Insurance is covered, provided that a record of all trips taken including the names and ages of all travelers must be submitted to our office.
US Activities:
Playing in U.S. covered?
Your Team is covered when playing in the U.S. against other teams, for example in an U.S. Tournament. We do not cover non-Canadian teams and players. However, if your team regularly plays in regular games of a U.S. League, your team will not be covered.
Hold Harmless Form:
What is the Hold Harmless Form?
In participating of any sports activities, there is always chance of getting injured. Whenever players register to participate as part of your league organization, it will be appropriate in bringing the risks to the attention of the players, parents or guardians of the registering player. Most organizations have their "Hold Harmless" as part of the Players' Registration Form, or the following samples can be used for your reference: Sample Player Registration Form.pdf
Note:
A sample copy of this Hold Harmless Form, or your Player Registration Form including this "Hold Harmless Notice", must be submitted to our office to complete your Registration and Application for insurance.
