With this online system, you are able to submit and process your registration record by yourself, such as adding a team, change your contact information, and even print your own insurance certificate.

Available Coverage for Application and Registration for Insurance:

(1) Liability & Accident Benefit (premium charged is based on the number of registered teams for your League):

• Complete Online Application Form;
• [Submit] Application Online.

You will be notified by email for further detail on your insurance registration process.

(2) Directors & Officers Liability (No additional premium charge with the above Registration):

Coverage for Directors & Officers Liability will not be provided if the following are not received by our office.

• A copy of your League’s recent “Financial Summary” indicating revenue and expenses; and
• A list of your present Directors and Officers

(3) Travel Insurance (No additional premium charge with above Registration):

The following information must be forwarded to our office prior to the departure date, and Travel Insurance coverage confirmation will be provided to you by email.

• Destination of Travel;
• Date of Departure and Return;

• Listing of the traveling players (including Date of Birth).
